关于教育部要求核查学生信息的通知

各位老师、学生和家长：

大家好！按照教育部规定，每年每位在社区语言学校学习的学生的信息必须由所在社区语言学校上报。学生信息每年必须及时更新，作为教育部对社区语言学校的审计、经费补助以及学校购买保险和发生紧急事故查询的依据。学生信息由专职老师负责并接受教育部监管。

我校从本周起会通过各班班主任老师向每一位学生发放打印好的2021年学生信息表，表上的内容符合教育部学生信息资料库和对社区语言学校审计的要求，请各位家长配合我们核查表上的信息是否准确。凡是原表内容不清晰或有内容遗漏的，我们会请家长重新填写一张表格，请谅解！

在核查信息时，请大家对以下几项内容要特别注意：

第一，除中文姓名外，其余内容均用英文大写字母填写的；如无中文名，可不填；

第二，学生的姓名要跟护照或出生证明上的名字一样，需全部填写；

第三，学生的生日请按“日/月/年”填写；

第四，学生就读的日校如有不同校区的，请注明具体哪一个校区；

第五，学生的年级请按日校的年级填写。

我校历年学生信息收集和核查工作都做得很出色，这是全校老师、学生和家长负责和高效合作的结果。我相信今年我们会做得更好。请家长核查后尽快反馈给班主任老师，以便学校及时上报教育部。

Notice on completing the Student’s Information sheet

Dear parents,

As required by the Department of Education & Training (DET), any community language school is expected to submit the information of all the students enrolled in Term 1 for the purpose of the school’s accreditation, funding application, purchasing insurance for the students and emergency contact and the information is to be updated annually. The information collected would be saved securely in school and supervised by DET.

The printed Student’s Information sheet you’ve received this weekend is for your 2021 enrolment based on the requirements of student database and school auditing process. Please double-check whether the information on the sheet is accurate and updated. If the original sheet is not clear or not completed, we might ask parents to fill in a new one.

When you check the information of your child, please pay attention to the following issues:

  1. Complete the information sheet in BLOCK letters expect for the Chinese name. If your child does not have any Chinese name, leave it blank;
  2. Please make sure your child’s full name is as what is shown in his/her passport or birth certificate;
  3. Please make sure the format of your child’s date of birth is as dd/mm/yyyy;
  4. If your child’s day school has more than one campus, please indicate the campus name of the day school your child has enrolled in;
  5. Your child’s year level refers to your child’s day school year level.

We have done great jobs in the past years collecting and checking students’ information thanks to the cooperation among teachers, students and parents. I believe we can make it this year as usual. Please inform your child’s class teacher about the correctness of the information as soon as possible.
